Appendix B contains blank header comments you can use as templates to add standardized comments to your programs.
These templates are described in Chapter 7, "Comments." Click here to download a zip package

Dave Pettit has expanded the form templates to include more information
describing what should be included where. The comments remind you about what to put in each section. Normally you
would fill in the information and then remove these reminder comments. Click here to
